Doctor Locator Instructions

STEP 1

CLICK HERE to access DOCTOR LOCATOR in a new window. You will see a window similar to the graphic as shown on left.


STEP 2

Enter your ZIP CODE in the box marked and click on SEARCH. This will take you to page as shown on left.


Double click on map near your area to zoom in, or adjust the magnification slider bar up or down on left of window along with the navigation arrows above slider bar..


STEP 3

As you continue to zoom in, PURPLE and GREEN flags will begin to appear in geographical areas.


NOTE: LEGEND key below map depicts what each flag indicates:

PURPLE FLAG WITH STAR: Impulse® Doctor.
PURPLE FLAG WITH Q: iQ® Doctor.
GREEN FLAG WITH STAR: CERTIFIED Impulse® Doctor.

GREEN FLAG WITH Q: CERTIFIED iQ® Doctor.


STEP 4

When you have located your icon, it should appear similar to example on left.


Keep selecting flags until you find your listing. If you do NOT find your flag and listing, please CONTACT US to resolve the issue as soon as possible.


STEP 5

When you locate your listing and flag, click on VIEW PROFILE. The page should appear as on left.


If your flag is not the correct denotation according to the Legend, please CONTACT US to resolve the issue as soon as possible.

STEP 6

When you select VIEW PROFILE, the screen at left appears.


Your USERNAME is your EMAIL ADDRESS.


Enter your PASSWORD.


If you do not have or know your password, select the "Click here" link next to "Forgot your password?" text. The system will email you a new password.


Return to this login page and enter your EMAIL ADDRESS and NEW PASSWORD. If you are still unable to log in, please CONTACT US to resolve the issue as soon as possible.

STEP 7

The next screen, SEARCH FOR AN IMPULSE DOCTOR, appears as shown on left. Please fill out all relevant fields. Include all OFFICE HOURS, SERVICES OFFERED, TECHNIQUES OFFERED and your BIOGRAPHY.


To upload images: Select BROWSE button below UPLOAD PICTURES section on right to upload your digital image.


Select SAVE CHANGES at bottom.

STEP 8

After selecting SAVE CHANGES, a screen similar to the image on left should appear.
