Your first step is to complete the inspection/repair service request form below. Please fill out as much information as possible so our service and practice advisor team can assist you in the most efficient manner.
Repair and Service Options
Due to FDA and Quality Management standards, instruments that have been in service for greater than 5 years will not be eligible for service repair.

Due to FDA and Quality Management standards, instrument that have been in service for greater than 5 years will not be eligible for service repair.
Instruments may be returned directly to Neuromechanical Innovations. Upon delivery the service team will inspect your instrument and identify a resolution. If your instrument is eligible for warranty repair, it will be repaired and returned to you.
US customers will be charged $39 for return shipping.

Due to FDA and Quality Management standards, instrument that have been in service for greater than 5 years will not be eligible for service repair.
“Provisional Instrument” Select this option if you would like an instrument shipped to you for use while your instrument is in for inspection/repair under warranty. Provisional Instruments are grade A+ condition with little use in training or at our research center.
a. You will be charged a Provisional Instrument Deposit of apporximatley 50% of the instrument retail price as listed in the online store, shipping to you office and shipping back to NMI.
b. Upon the arrival of the Provisional Instrument, package and return the instrument requiring service to Neuromechanical Innovations.
c. Once your instrument has been inspected and repaired, NMI will ship the instrument back to you.
d. Upon the arrival of your repaired or serviced instrument, package and return the Provisional Instrument to NMI.
e. Once your defective instrument has been returned and NMI has received the Provisional Instrument, NMI will refund the Provisional Instrument Deposit.
f. After you have received your repaired instrument, NMI may charge your credit card for the advertised full retail value of the Provisional instrument if the Provisional Instrument is not returned to NMI within 30 days.

“Replacement Instrument” This option requires pre-authorization from the NMI service center or your NMI Practice Advisor. NMI will ship a Replacement Instrument that has been quality inspected and in grade A+ condition with little use in training or at our research center.
a. You will be charged a Replacement Instrument Deposit.
b. Upon the arrival of the Replacement Instrument, package and return the defective to Neuromechanical Innovations (NMI).
c. Once your defective instrument has been returned and received at NMI, NMI will deduct shipping and handling fees, and return the balance of your deposit.
d. If the defective instrument is not returned and received by NMI within 30 days of the replacement instrument delivery, NMI may charge your credit card for the advertised full retail value of the replacement instrument.
